How does Vericare measure the progress of their clinical work with elderly residents?

  • Vericare providers present a structured patient update program to the administrative staff at the skilled nursing facility on a quarterly basis. Our clinicians share neurological assessment, treatment planning and patient progress in a formal review meeting. These 12-week summary meetings provide a progress report on all patients and include assessment, treatment planning, and visits notes that emphasize the outcomes (both short and long-term) of all cases.
  • Vericare Regional Managers perform routine, on-going Quality Assurance on all Vericare psychologists, psychiatrists, LCSWs and advanced practice nurses providing behavioral health services. Vericare’s Behavioral Management Quality Assurance Program is designed to ensure that behavioral objectives are met and treatment plans are implemented for each Vericare patient.
  • Each provider, during the course of treating patients, completes “Patient Notes” to update the patient's care team on therapeutic goals and progress towards these goals.
  • Vericare Regional Managers, in concert with the Chief Clinical Officer, reviews these notes to ensure that the patient is making progress towards their agreed upon goals as outlined in their treatment plan.
  • The Regional Manager and Chief Clinical Officer also follow-up to ensure that each skilled nursing facility is being formally updated on their patients’ treatment plans and progress on a quarterly basis.
